...an interactive learning environment designed to support knowledge management within women’s services. Our purpose is to help create ‘an organisation with a memory’ so that important lessons from sources such as: incident reviews, complex clinical cases, national audits and examples of good and bad teamworking are fed back into the system to increase system resilience and improve the quality & safety of patient care.
Overall, our two main aims are:
1. IMPROVE LEARNING FROM ERRORS TO TRY AND PREVENT
THE SAME THINGS HAPPENING AGAIN (Safety 1)

2. IMPROVE LEARNING FROM WHEN THINGS GO RIGHT, SO THAT WE
CAN ENSURE THEY GO RIGHT MORE OFTEN (Safety 2)

In this area, you will find a series of interactive learning summaries which can be used for individual learning or as a focus for mini-simulations and / or group discussions. Based on the principle of ‘continuous training’ i.e. that that knowledge and skills training should be undertaken regularly to be effective (much like training the body) we aim to release a new topic every week. The topic will be shared on the SWITCH website & via social media and will also form the basis of a weekly simulation or group discussion. More detailed topics will be covered in longer ½ day sessions once a month.
